Dear client, for the safety of the traveler/s, if you’re only requesting to handover the escortee/s to any designated person at final destination then please fill this form. The assigned travel chaperone will ONLY handover the escortee/s to the person/s indicated in this form. It is the responsibility of the client to advise the person receiving the escortee/s that he / she must show a valid government photo ID to the assigned travel chaperone, and his / her name will be matched with the name/s provided by the client on this form as well the receiving person he/she must sign & date this form.
